Request a repeat prescription

Prescriptions

We can now send your repeat prescription electronically, directly to your pharmacy. In order to do this we will need the following:

1. Your name and date of birth.

2. List of medications requested and for length of time, ie one month, three months, six months.

3. Your chosen pharmacy name and address.

Once we receive your order the pharmacy will receive the prescription 48 hrs later.  Please email us the request or drop in a list to reception.

Please be advised of €20 fee for private prescriptions & continuing medical certs.

We will contact you for payment when this is being processed.

To request a repeat prescription or social welfare certs please choose one of the following.

Email your request to: cambridgeclinic@gmail.com or 

Drop in an up to date list of medications you require in writing with your name, date of birth & pharmacies name and address. 

Your prescription will be sent electronically to your pharmacy 48 hours later for collection there.

If you are attending a consultant for follow up and require a copy of your results please contact reception and we will leave a copy out for you to collect. Due to GDPR regulations we can no longer email any sensitive information ie results, referral letters etc. Please note there is a 5/10e charge for printouts.


GMS (Medical cards)

If you wish to avail of this service you must have your medical card with you for your visit and in date. It is up to you to keep your card in date and you can do this by contacting the HSE . You can also call in to them or telephone them at 01 834 3644 or log on to their website www.hse.ie If you present to the surgery and your card is out of date/invalid you will be charged a consultation fee for your visit.

If registering to the surgery as a new patient for the first time and you have a medical card with another doctor you must sign a “transfer of doctor form” if you want to continue as a patient at our surgery as you can only be registered to one doctor. Please ask the receptionist for this form. When this is processed and you receive your new card you may then make an appointment to see one of our doctors.

Getting to the Cambridge Clinic

From city centre

Coming from the city centre continue through lower Rathmines to the Upper Rathmines Road passing Tesco supermarket on the right hand side. Continue straight on and take the first turn to the left on to Church Avenue. You will see a church known as the “Black Church” straight ahead of you on the centre of the road. Our surgery is to the left hand side of the church. It is the first house on the left hand side with a red door no.5. Please use the side entrance to the left side of the house.


Parking:

There is disc parking available outside the surgery.

Wheelchair access:

Please come to the main door to the front of the house where we have a ramp provided.  Please let reception know you are waiting at front of house.
